Feedback and Complaints

If you wish to offer feedback or submit a complaint regarding the services provided by In Home OT, kindly complete this form. Please note that all feedback and complaints are private and confidential. As In Home OT functions as a sole trader business, all submissions will be sent to the owner, Joshua Harris. Once received, the owner will follow up with you and ensure your concerns are addressed, unless you ask us not too contact you. Please note that if you are making a complaint anonymously, we will not be able to respond to your complaint and let you know what actions we are taking to address it.
